Registration Forms & Fees

$425 if paid prior to 7/9/24
$525 if paid after 7/9/24
$350 each sibling
$400 equipment deposit ***returned end of season
$200 volunteer deposit ***returned end of season

5. Copy of medical insurance card
6. Copy of birth Certificate
7. Proof of passing (sports) physical within 6 months: Click here for IHSA Physical Form
8. Signed Sports Medicine and Acknowledgement: Click here for IHSA Medical Consent
9. Proof of immunizations or religious waiver
10. Copy of transcripts: Click here for transcript temple
Once all paperwork is signed and all the requirements are met above. Submit in person, or send all information to info@illinoiscrusaders.com
