Registration Forms & Fees
1. Payment
$525 Fee for the season
$350 For each sibling
$400 Equipment deposit ***returned at end of season
$200 Volunteer deposit ***returned end of the season
2. Crossroads Medical release waiver form: Click here for waiver
3. Intent Form
4. Player Contract
5. Copy of medical insurance card
6. Copy of birth Certificate
7. Proof of passing (sports) physical within 6 months: Click here for the IHSA Physical Form
8. Signed Sports Medicine and Acknowledgement: Click here for IHSA Medical Consent
9. Proof of immunizations or religious waiver
10. Copy of transcripts: Click here for transcript temple

*All paperwork and payments are required for players to get their equipment issued, and have full contact.
